    Requirements for Claiming Funds

     

    If the claimant is an individual, (without representation):

    • Completed application and order for payment of unclaimed funds
    • A certificate of service showing that notice of application has been given to the United States Attorney pursuant to Title 28 USC Section 2042. (See address list below.)
    • To provide proof of ownership of funds, attach a copy of drivers license or another acceptable form of identification, and send the Registry Fund Confidential Personal Identification form to the Financial Specialist in the San Antonio office.

    If claimant is represented by a funds locator:

    • An original power of attorney with a notarized signature.
    • Completed application and order for payment of unclaimed funds.
    • Evidence of business identification credentials, if creditor is a corporation.
    • Disbursements will only be made in the name of the creditor.
    • A certificate of service showing that notice of application has been given to the United States Attorney pursuant to Title 28 USC Section 2042. (See address list below.)
    • To provide proof of ownership of funds, send the Registry Fund Confidential Personal Identification form to the Financial Specialist in the San Antonio office.

    If claimant is business or corporation:

    • Completed application and order for payment of unclaimed funds.
    • Evidence consisting of Articles of Incorporation, Articles of Merger or Successor Corporation, etc. Representative of firm must be identified in evidence articles.
    • A photocopy of representative’s identification credentials.
    • A certificate of service showing that notice of application has been given to the United States Attorney pursuant to Title 28 USC Section 2042. (See address list below.)
